The program works alongside a patient’s regular retina visits rather than replacing them.

What Is Retina Care Plus?

Retina Care Plus is a Principal Care Management (PCM) program from Retina Specialty Institute. PCM is a Medicare-recognized care management service. It is designed for patients with a single, serious chronic condition expected to last at least three months. This type of condition carries a real risk of disease progression, functional decline, or hospitalization.

A traditional office visit focuses on the time you spend with your physician during a scheduled appointment. Retina Care Plus focuses on the time in between. It gives our Gulf Coast team a structured way to stay connected between visits. We can answer questions and help you manage your condition day to day.

How Retina Care Plus Works

  1. At Your Visit. Your physician reviews your condition and determines whether Retina Care Plus may benefit you. If it’s a good fit, we’ll enroll you in the program
  2. Between Visits. Once enrolled, our team calls you monthly to answer questions, review your treatment plan, and provide support.
  3. Ongoing Care. You continue seeing your retina specialist as scheduled. Retina Care Plus works alongside your regular appointments, not in place of them.
